Notice for U.S. Immigrant Vaccination

• Official time:  

  08:00-11:00a.m., Monday to Friday. 

  Applicants under 18 years old shall be accompanied by their legal guardians. 

    

  Address:  

  Shanghai International Travel Healthcare Center.   

  2^nd floor, Building 3, No.15 Jinbang Road, Changning District, Shanghai.  

    

  Medical fees:  

  It is advised to bring 2000 RMB of each applicant for health examination and vaccination.  

  It is advised to bring 600 RMB for booster shots.  

    

  Documents:  

  1.Applicants applying for the first shot:  

  ①Valid original passport.  

  ②Original vaccination record (childhood record, if you have).  

  2.Applicants applying for the booster shots:  

  ①Valid original passport.  

  ②International Vaccination Certificate.  

  ③A photograph of two inch size.  

    

    

    

  Applicants include：  

  U.S.immigration vaccination applicants include those who have been approved and have the valid interview letter from the U.S. Consulate General in Guangzhou or the U.S. Embassy in Beijing or U.S. Citizenship and Immigration Services.   

  Including:  

  1.Applicants apply for the first shot.  

  2.Applicants apply for the booster shots according to the criteria of the U.S. CDC and general consulates.  

    

  Notice:  

  Applicants for vaccination do not need to be fasting.
