【Applicants】
Physical examination applicants are the four kinds of people below, who are applying for living, working or studying for a year or more:
(1)Foreigners
(2)Chinese citizens (Hong Kong and Macao residents)
(3)Chinese citizens from Taiwan Province
(4)Chinese mainland citizens settling overseas
The regulation doesnt apply to children under 16.
【Address】
Shanghai International Travel Healthcare Center. Ground floors of both Building 1&3, 15 Jinbang Road, Changning District, Shanghai.
【Service Time】
08:00-11:00, Monday to Friday.
Besides, based on the number of applicants, we provide extra service on the afternoons from Monday to Thursday, and Saturday flexibly.
【Reservation】
Applicants should make reservations in at least two weeks advance.
【Documents】
1.Foreigners：
(1)A valid original passport or travel document, and photocopies of the photo-page, visa-page and info.-update-page of your passport or travel document;
(2)Three recent 2-inch photos (full face without hat);
(3)Please fill in the Consent Form to us. （download ,Please print the form and sign it）
2.Chinese Citizens（Hong Kong and Macao residents）：
(1)A valid original home return permit or travel document and photocopies of the photo-page, visa-page and info.-update-page
(2)Three recent 2-inch photos (full face without hat)

3.Chinese Citizens from Taiwan Province：
(1)A valid original Taiwan Compatriot Travel Certificate or travel document and photocopies of the photo-page, visa-page and info.-update-page
(2)Three recent 2-inch photos (full face without hat)

4. Chinese mainland citizens settling overseas：
(1)A valid original passport or travel document and photocopies of the photo-page, visa-page and info.-update-page of your passport or travel document
(2)Three recent 2-inch photos (full face without hat)

【Relative Documents】
1. Applying for working visa: an original and photocopy of the certificate issued by SAFEA and company's business license or registration certificate.
2. Applying for studying visa: an original and photocopy of students ID or admission notice.
3. For accompanying family members: a photocopy of family members' business license.
4. For visiting, house-purchasing, marriage, medical treatment or other purpose: an original and photocopy of real estate certificate or registration form of temporary accommodation issued by police station affiliated to the local community.
5. If you have an overseas health check report issued within 6 months, please bring all the original documents including Chest X ray film and laboratory result.
【Attentions】
(1)Please keep the stomach empty for at least 5 hours. (Except for the water)
【Certificate-collecting】
There are two ways to collect your certificate:
1.In person.
In four working days after the examination, applicants can come to the certificate-collecting window in person with valid IDs and the examination receipts
2.By courier.
After the examination, applicants can also sign up for courier services to deliver your certificates to the appointed addresses after four working days.
To ensure the safety and privacy of your examination certificates, applicants must go to the certificate-collecting window in person to change the way of collecting your certificates.
【Reexamination】
We will send a notice to those whose results are abnormal. Upon receiving the notice, applicants should go to the appointed department for reexamination within the stated period of time with valid IDs and the examination receipts.
